- Individual members who serve on the staff or Board of a NACHC Organizational Member may also serve on NACHC committees, run for NACHC office, and qualify for the NACHC Achievement Award.

Full Individual Membership:
Dues: $65 per year - includes a subscription to the Community Health Forum™ Magazine
Basic Individual Membership:
Dues: $30 per year - does not include a subscription to the Community Health Forum™ Magazine
Full Lifetime Membership:
Dues: $325 (one time fee) - includes a subscripiton to the Community Health Forum™ Magazine.
