Organizational Members “in good standing” will receive an Express Credentials Certificate by mail the week of August 25th. The Executive Director, Board Chair, or the health center's official designee by proxy (see Voting by Proxy below) may proceed directly to the Credentials Booth to present their certificate and retrieve delegate materials for voting.
Who can pick up the delegate packet?
- The Executive Director or the Board Chair is eligible to pick up the health center’s credentials at the Credentials Booth.
- If both the Executive Director and the Board Chair are unable to attend the annual meeting:
- The health center should, prior to the meeting, designate an individual to credential the health center on-site, and advise all health center board members and staff whom that individual will be.
- At on-site credentialing, the health center’s designee must present the 2008 Proxy Authorization Letter on health center stationary, signed by the Executive Officer or Board Chair, stating the designee’s name and health center (see Voting by Proxy below).
What if a health center changed their name?
- The NACHC Membership Department must be notified, in writing, of any health center name changes prior to credentialing. Only the health center, as named in NACHC records at the time of credentialing will be accepted; NACHC must receive the notification in time to change the records.
Voting by Proxy
An eligible center unable to attend the House of Delegates can vote by proxy.
- Under NACHC Bylaws, Organizational Members “in good standing” not sending the Executive Director or Board Chair to the Annual Meeting, but wishing to vote, may authorize an individual to hold its proxy. Authorization is granted by sending a letter on the Center’s stationery, signed by the Board Chair or Executive Director, to the Credentials Committee stating the name of the person who will be holding the proxy.
- An Organizational Member “in good standing” may authorize only one proxy (which carries four [4] votes) on its behalf.
- There is no limit to the number of proxies an individual can be authorized to hold.
- Proxies are not transferable. The designated person cannot give the proxy to another person.
