This CoP is designed to assist CHCs in better understanding the principles of patient engagement and to offer tools and strategies for effective engagement. There will be three sessions dedicated to using patient portals, gaining the trust and participation of patients and working with difficult population groups. This will be an excellent opportunity to improve the utilization of a patient portal and begin ramping up to meet the patient engagement requirements of PCMH and MU Stage 2.
